Linaclotide, also known as Linaelotide acetate under the trade name Linzess, is a guanylate cyclase-C receptor agonist (GCCA) with both visceral analgesic effects and endocrine activity.
Linaclotide and its active metabolites can both bind to guanylate cyclase-C (GC-C) receptors on the luminal surface of the small intestinal epithelium. Through the activation of GC-C, intracellular and extracellular cGMP (cyclic guanylate) concentrations rise. Extracellular cGMP, which reduces the activity of pain nerve fibers, reduces visceral pain in model animals. Intracellular cGMP can increase the amount of chloride and bicarbonate secretion in the small intestinal lumen by activating CFTR (cystic fibrosis transmembrane conductance regulator), ultimately leading to increased small intestinal fluid secretion and faster small intestinal transit.
